- The Project Form

Every project, company or billing entity must complete a separate Nanotech West Laboratory Project Form that gives Nanotech West essential information on the project's authorizer as well as billing contact and address. Each project will then receive a separate account number. (OSU account numbers coincide with Research Foundation project numbers.)
- OSU Faculty, Staff and Students
OSU Faculty, Staff and Students must provide chartfield information consisting of an organization number, fund number and usually a project or program number to which lab fees are charged. Lab fees can be found here. The charges are billed through a journal entry or interdepartmental billing in the University's online Financial System and an invoice is sent to the billing contact afterwards. Your department may require that a 100W be on file with Nanotech West for their records but we consider the project form to be authorization to charge the chartfield.
- Non OSU Users
Non OSU users must provide Nanotech West with any essential documentation, such as a purchase order, required by your organization for them to authorize payment of the invoice when it is received. Currently, we can only accept checks for payment of an invoice. All invoicing is done after the first of the month for the previous month's lab use. Lab fees can be found here.
Note: Federal guidelines require separate user fee schedules for academic institutions and non-academic institutions.
- The User Form

Complete a Nanotech West User Contact Form. This has general and emergency contact information that we require and the protocols agreement you will sign in order to have access to the lab. Completion of the user contact form requires company or PI authorization.
- Safety Training
All users must complete safety training through one of the following options:
- EHS Online training
Complete the online OSU Environmental Health and Safety (EHS) safety training modules listed below and pass the associated online tests. Training modules can be accessed through the EHS website and you will have to register on the EHS WebPass system. In total these modules will take approximately 3 hours. If you already know the material, you can proceed immediately to the "Test Out" option and complete the modules in about 20 minutes. The training modules and tests do not have to be taken all at once. Please notify Paul Steffen upon completion.

| Compressed Gas Cylinder Safety | 10 | Only staff are permitted to handle compressed gas cylinders at this time. |
- OSU Chemistry 685
Provide the academic quarter (example: Winter 2005) during which you completed the Chem 685 Safety Training Seminar course at Ohio State.
- Orientation & Safety Tour
Attend a Nanotech West Laboratory Orientation and Safety Tour. Tours are conducted on a regular basis and can be scheduled through Mary McCleery. The tour includes an introduction to the building and an explanation of safety information specific to Nanotech West Lab. The tour for BioHybrid Lab users is 60 minutes and the tour for Cleanroom Lab users is an additional 30 minutes. Upon completion of the tour cleanroom users will be assigned storage locations for cleanroom togs and a blue storage bin.
- Lab Access
After completion of the Orientation & Safety Tour and signing a Key Fob Agreement, users will be given an electronic key fob that gives them access to areas they have been trained to use. This fob is only to be used for the user's personal access to the labs.
- Specific Tool Training
Most tools and processes require additional training and certification. The list of all major tools and capabilities and the Nanotech West staff member assigned to each tool may be found here. Contact the primary staff member listed to schedule specific tool training. If you are unsure what tools or processes you will need for your research, then please contact a member of the staff.
